Acid-Base Balance

Body fluids must maintain a normal acid-base balance in order for normal cellular function to sustain health and life. Acid-base balance can be described by measuring the pH (or acidity) of a substance on a scale that ranges from 1 to 14 .1,4 Normally, blood has a pH of 7.35 to 7.45. A pH value outside this range indicates a serious acid-base imbalance. The body has numerous compensatory mechanisms to correct an abnormal pH; however, if these mechanisms fail (Figure 4Go), cellular functions are impaired, and death will eventually result.

Acidosis

Metabolic alkalosis occurs when body fluids contain an excessive amount of bases or a deficit of acids and can be diagnosed on the basis of the increased bicarbonate level or base excess in arterial blood gas sampling

Compensatory Mechanisms
The ratio of bicarbonate to carbon dioxide is 20:1, and this ratio is normally maintained through compensatory mechanisms or chemical buffers present in the extracellular fluid, body cells, blood cells, and plasma. Buffer systems maintain acid-base balance in 2 ways: by correcting or altering the component responsible for the imbalance and by compensating through alterations in the component that is not primarily responsible for the imbalance. These buffer systems can act within a fraction of a second to prevent excessive changes in pH

Oxygen Administration Devices Summary

Oxygenation –is a basic human need. Breathing is synonymous with life. The respiratory system replenishes the body’s oxygen supply and eliminates waste from the blood in the form of carbon dioxide. Oxygen is colorless odorless, tasteless and dry gas that supports combustion.

High Flow Administration devices

1. Venturi mask/Low-concentration venture – type mask is proffered for client with COPD because it provides accurate amount of oxygen. They required 2-3 L/min or 28% oxygen.
2. Face mask
3. Incubator/isollete – Can be used for low and high flow concentration.
4. Oxygen hood – Can be used for low and high flow concentration.

Low Flow administration devices

1. Nasal Cannula (24-45% at 2-6L/min. May be used in clients with COPD at 2-3 L/min if venture mask is not available
2. Simple Face Mask (40-60% at 6-10 LPM)
3. Partial Rebreathing Mask (60-90% at 6-10LPM)
4. Non-rebreathing Mask (95-100% 6-15LPM)
5. Croupette
6. Oxygen Tent

Nursing Intervention

1. Assess signs and symptoms of hypoxemia
2. Check doctor’s order
3. Position the patient preferably semi-fowler’s position.
4. Open source of oxygen before insertion of oxygen device.
5. regulate oxygen flow accurately
6. Place a “No-smoking” sign at the bedside. Strictly enforce this warning
7. Avoid use of oil, greases, alcohol and ether near the client receiving oxygen
8. Place sterile water into the oxygen humidifier
9. Provide good oronasal hygiene to prevent dryness and irritation of mucous membrane
10. Lubricate nares with water soluble lubricant to soothe the mucous membrane. Do not use oil.
11. Assess the effectiveness of oxygen therapy.
12. Check vital signs. Especially RR; note quality of respiration; evaluate arterial blood gas results (ABG analysis)
13. Document the procedure

Mechanics of Ventilation

Ventilation in the physiological sense (strictly, pulmonary ventilation) refers to the volume of air breathed in and out of the lungs: typically 5-6 litres per minute for an average-sized person at rest, with the possibility of increasing by up to twenty-fold in exercise.

During Inspiration, the diaphragm contracts and flattens in order to expand the thoracic cage and increased the cavity of the thorax. Pressure in the thorax decreases to a level below atmospheric pressure. This time air moves into the lungs.

After a complete inspiration, the diaphragm relaxes, and the lungs recoil to its normal position, the size of thoraxic cavity decreases, increasing pressure to levels greater than atmopheric pressure. Air then flows out of the lungs into the atmosphere.
